Awards are presented each year to alumni, faculty, staff, preceptors and students who have shown a strong commitment to community service, leadership and overall involvement in both the NEOMED and Northeast Ohio communities.

The nomination forms lower on this page are broken down by nominators: student (organized by cohort), faculty, staff, preceptor and alumni. The list of awards is located within each drop-down menu. To nominate an individual for an award, click on the survey button within the drop-down.

Alumni Nomination Form

This is where alumni can nominate individuals for awards. Alumni can nominate for the following:

College of Pharmacy Distinguished Alumni Award: Given to an alumnus/alumna who has made outstanding contributions to the pharmacy profession.

Qualifiers

Nominees must be an alumni and meet at least 2 of the 3 following criteria:

  • Exemplifies outstanding leadership and serves as a role model for others.
  • Actively contributes to the pharmacy profession through his/her practice site, research and/or involvement in professional organizations.
  • Contributes to the NEOMED College of Pharmacy (e.g., preceptor, on-campus teaching, committee involvement, etc.).

 

Excellence in Service: An individual who has a record of professional and/or extra-professional activities to advance the practice of pharmacy, and/or contributions of time, talent or financial support to benefit NEOMED.

Faculty Nomination Form

This is where faculty can nominate individuals for awards. Faculty can nominate for the following:


Student Awards

  • Student of the Year: Given to a student who exemplifies the core values of NEOMED including Leadership, Excellence, Advocacy, Diversity, Equity, and Respect
  • Leadership & Advocacy: Given to a student who has exceeded expectations in a student leadership role and/or who has taken special initiative to focus on one or more pharmacy advocacy activities
  • Excellence in Research: Given to a student who has pursued one or more scholarly activities beyond the requirements of academic research endeavors which have contributed to the advancement of the profession of pharmacy

Faculty Awards

  • Excellence in Scholarship Award: This award is given to a faculty member who has made substantial scholarly contributions to his or her field in the past year. The nature of the research may be basic, translational, clinical, or in the realm of teaching and learning.
  • Faculty Excellence Award: A faculty member who has demonstrated excellence in the areas of teaching, scholarship, and service, and who exemplifies the core values of  NEOMED including Leadership, Excellence, Advocacy, Diversity, Equity, and Respect.
  • Innovation in Teaching Award: A faculty member who has developed and/or applied a teaching strategy, approach, technique, or tool that is used to improve learner outcomes or the learner experience
  • Faculty Diversity Award: This award is given to a faculty member who has distinctive and outstanding teaching, research, and/or service that has advanced diversity and equality at NEOMED.

Staff Awards

  • Staff Excellence Award: A staff member who consistently and substantially exceeds the expectations of the position, performing at a level above and beyond normal job requirement and has made important and significant contributions in their area
  • Staff Diversity Award: This award is given to a staff member who has demonstrated, through his or her positive interactions with others, a respect and value for diversity in the NEOMED community.

Preceptor Award

Interprofessional Practice Award: Please nominate a faculty member or preceptor who exemplifies interprofessional collaboration through one or more of the following:

  • Demonstrates enthusiasm, commitment and knowledge of interprofessional collaboration.
  • Demonstrates effective communication skills with learners, colleagues and/or patients.
  • Actively and consistently collaborates with other health care professionals to model interprofessional collaborations.
  • Advances practice/research in interprofessional collaborations

P1 Preceptor of the Year: Please nominate a preceptor who has

  • Served as an excellent teacher and role model for a minimum of three students in the corresponding class awarding the honor
  • Received positive experiential evaluations from a minimum of two students in the corresponding class awarding the honor
  • Demonstrated professionalism in all interactions with students, staff and patients
  • Contributed to the professional development of the students and advanced the profession

Interprofessional Practice Award: Please nominate a faculty member (core or volunteer) who exemplifies interprofessional collaboration through one or more of the following:

Hooder Nominations: The Commencement Hooders will bestow the doctoral hoods during the commencement ceremony.

Most Influential Professor/Faculty Award: Given to a faculty member who has worked with students and is dedicated to their education and professional development.


Student Awards

Student Excellence in Role Modeling of Interprofessional Collaboration Award: Given to a graduating student who exemplifies interprofessional collaboration through one or more of the following:

  • Demonstrates enthusiasm, commitment and knowledge of interprofessional collaboration
  • Demonstrates effective communication skills with peer learners, colleagues and/or patients.
  • Actively and consistently collaborates with other health care professionals to model interprofessional collaborations.
  • Advances practice/research in interprofessional collaborations.

Bowl of Hygeia Award: Given to a classmate who you would most want as your own personal pharmacist as exemplified through excellent intellectual and clinical skills, empathy and patient rapport, as well as diligence and support of fellow colleagues.

Summa Health Rootstown Pharmacy Community Practice Award: Given to a classmate who demonstrates academic excellence and a strong commitment to community pharmacy practice.

Council of Ohio Colleges Pharmacy Leadership Award: Given to a classmate who is involved in leadership roles on campus, exemplifies the core values of NEOMED including Leadership, Excellence, Advocacy, Diversity, Equity, and Respect, is a role model for other students, exhibits professionalism, and is a positive ambassador of the College of Pharmacy

Professional Achievement Award: Given to a classmate who exemplifies scholastic and professional achievement, leadership ability, and ethical conduct.

Evidence-Based Medicine Approach to Patient Care: Given to a classmate who is academically strong, intends to enter practice upon graduation, demonstrates high personal motivation and a unique ability to communicate drug information.

Viatris Excellence in Pharmacy Award: Given to a classmate demonstrating promise in improving care and an appreciation for scientific inquiry and an evidence-based approach to evaluating natural medicines and a strong interest in an evidence-based approach to patient care.

Ohio Pharmacists Association Student Leadership Award: Given to a classmate who is a current member of APhA-ASP, demonstrates leadership and outstanding achievement in ASP chapter activities, and for recognizing the value of associations in improving the profession.

Outstanding Researcher Award: Given to a classmate who has demonstrated pursuit of research projects, presentations and publication of research results.

Excellence in Clinical Communication and Patient Care Award: Given to a classmate who demonstrates superior achievement in patient care and as a health care provider by means of superior verbal and written communication skills and effectiveness of relaying pharmacy knowledge.

Commitment to Service Award: Given to a classmate who has made outstanding contributions to the College of Pharmacy, his or her class and community through leadership, donation of time to class, and service and advocacy in community service programs.

Akron Area Society of Health-System Pharmacists Pharmacy Practice Award: Given to a classmate demonstrating academic excellence and strong commitment to health system pharmacy practice.

Class Gift: It is customary for the graduating class to present the College with a class gift, which would be purchased with class funds. Please provide any ideas for this permanent gift from the Class. Remarks will be communicated to your student council representatives. Examples include contributions toward the Tahani Mansour COP Memorial Scholarship, Pharmacy Emergency Foundation Loan Fund, General Pharmacy Scholarships, Pharmacy Priority Fund, COP Scholarship Fund, Wasson Center.


Preceptor Award

P4 Preceptor of the Year: Given to a core and a voluntary faculty preceptor who served as an excellent teacher and role model and demonstrated professionalism during the P4 year.

Innovation & Entrepreneurship: Given to a student who has proposed/implemented an innovative project or business idea
