Financial Aid

application forms

The following application form(s) are required to be considered for federal financial aid:

The Free Application for Federal Student Aid (FAFSA)

Students (and parents) must have a Federal Student Aid ID (FSA ID) prior to completing the FAFSA. Create an FSA ID and then complete the FAFSA at https://studentaid.ed.gov/sa/fafsa or on the myStudentAid app. NEOMED’s Title IV code is G24544. We strongly recommend using the IRS Data Retrieval Tool to transfer IRS data. This will ensure accuracy and streamline the application process.

Request for Special Circumstance

The U.S. Department of Education permits financial aid offices to review and approve amendments to a student’s FAFSA if there are mitigating financial circumstances not represented on the FAFSA application. Students should review the Request for Special Circumstance Policy to see if they qualify for a review of their FAFSA data and complete the Request for a Special Circumstance Form when submitting their appeal.

The following Form(s) are required to be considered for NEOMED-Based Scholarships:

NEOMED Financial Aid Supplemental Application

The NEOMED Financial Aid Supplemental Application is a data collection form the Office of Financial Aid uses to help determine eligibility for financial aid administered by NEOMED. Students review and answer a series of questions to determine qualification for NEOMED Foundation scholarships on the financial aid home page on Student Self-Service.

Strongly Recommended Forms:

In addition to completing the FAFSA, students are strongly encouraged to review and complete the following consents on the financial aid home page on Student Self-Service. Students will be provided login information for Student Self-Service once they have been admitted.

Consent for Electronic Communications, including text message notifications

The preferred method of communicating financial information to students is through electronic means.  Financial information disclosed through email or Student Self-Service includes, but is not limited to financial aid offers, student authorizations, missing application notices, direct loan notifications, and consumer information. Information sent by text includes reminders to apply for financial aid and important reminders regarding scholarship applications.

We encourage students to consent on Student Self-Service to use electronic communications, however this is a voluntary selection.  If we do not receive consent, then all communication regarding financial information will be sent via USPS standard mail to the local address on file.  In the event a local address is not provided, we will use a permanent address.  Students are responsible for updating their address and cell phone number in Student Self-Service.

Your decision on consent for electronic communication and text messaging can be completed on Student Self-Service.

Title IV Authorization

Federal regulations allow schools to use federal financial aid to pay for tuition and mandatory fees related to degree programs. However, students must authorize the use of federal student aid to pay for other charges such as: medical insurance, printing charges, parking, and overdue library fines, etc. This authorization excludes charges associated with certificate coursework.  Once authorization is given, students may contact the Office of Financial Aid at finaid@neomed.edu to review, change or rescind the authorization.

FERPA Release

In accordance with the Family Educational Rights and Privacy Act (FERPA) we protect the privacy of student education records by requiring prior written consent before disclosing specific details about financial aid and tuition statements to anyone other than the student. Students may complete the FERPA Release to specify the individual(s) with whom we may share protected information. We will not release information regarding your financial aid or student tuition account with anyone unless we have authorization to do so.

Students will need to confirm their identity prior to in depth counseling, either by showing their NEOMED or government issued photo ID, or by confirming personally identifiable information if counseling occurs over the phone.  Family members disclosed on the FERPA release will need to confirm demographic information of the student prior to the disclosure of FERPA protected data.

Securely Submitting Financial Aid Documents

While your application is being processed, you may be asked to provide additional documentation or information. Your financial aid may be placed on hold or withdrawn if the information requested from you is not provided in a timely manner. To ensure protection of your information, students who are asked to provide confidential information (social security number, tax information, etc.) to NEOMED should either:

  1. Hand deliver to the NEOMED Financial Aid Office during regular business hours (M-F from 8-5)
  2. U.S. Postal Mail to: NEOMED, Office of Financial Aid, 4209 SR 44, PO Box 95, Rootstown, OH 44272
  3. Securely upload a file through Student Self-Service. Select All Users from the menu, and then select Secure Document Upload. Choose type of document to upload, then press submit. A green box will appear in the upper right corner to let you know that your document was submitted successfully.
