Pathways to Pharmacy Forms

Welcome to the 2025 Pathways to Pharmacy Summer Camp! Below are the required forms that must be completed and submitted no later than Friday, May 30, 2025.

Choose the appropriate form, based on the participant’s age at the time of registration. Only one participation form should be submitted per participant:

Pathways to Pharmacy Form for Participants Under the Age of 18 – This form must be completed by a parent or legal guardian.
Pathways to Pharmacy Form for Participants 18 Years of Age or Older – This form must be completed by the Pathways to Pharmacy participant, if they are 18 years of age or older, at the time of registration.
