Bio
My name is Alexander Hoffman, Pharm.D., BCPS, CDCES. I earned my Doctor of Pharmacy degree from Ohio Northern University. I completed a PGY1 Pharmacy Residency at Kaiser Permanente Medical Plan of Ohio and served as a pharmacist for Kaiser Permanente before joining Northeast Ohio Medical University (NEOMED) and Healthspan Integrated Care in 2013.
Today I serve as a primary care clinical pharmacist at the Cleveland Clinic. I am a Certified Diabetes Care and Education Specialist (CDCES) and am Board Certified in Pharmacotherapy. I completed my Master’s in Education in Health Professions Education in 2021.
