Transitioning from Hospital to Home During COVID-19: Project ECHO Pop-Up Session

In response to the challenges faced in the care of older adults when transitioning from the hospital to home, Northeast Ohio Medical University and the Geriatric Resources Ohio Workgroup (GROW) plan to host a free, online pop-up Project ECHO® session Wednesday, May 6, from 12:30-2 p.m.
“Transitions Hospital to Home: Challenges in COVID-19 Pandemic” will feature a panel of experts who will share their perspectives from acute care, home care, primary care and Area Agency on Aging and discuss strategies to improve overall care for older adults.
