Biography
Danielle Taylor is a Clinical Lead Pharmacist in the ambulatory care setting at Summa Health Family Medicine Center, where she assists in chronic disease state management, transitions of care in the inpatient and outpatient setting, and serves as a faculty member to family medicine medical residents. As well as an Assistant Professor of Pharmacy Practice at Northeast Ohio Medical University (NEOMED) in Rootstown, OH.
She earned her PharmD from Northeast Ohio Medical University (NEOMED) in 2021 and expanded her clinical knowledge by completing a PGY-1 residency at Summa Health.
Research interest
Areas of interest include chronic disease state management and transitions of care. Past projects have included a pilot study in efforts to expand transitions of care programs. Current projects include a population health initiative for patients in our family medicine center with A1c >9%.
