Make a Care Team Referral

Care Team is a campus-wide network of resources that focuses on prevention, early intervention and provision of support services for individuals in the NEOMED community experiencing distress or engaging in harmful or disruptive behaviors.

The referral form, lower on this page, is a resource for anyone at NEOMED to communicate a concern about an emerging situation involving an individual faculty, staff, student or guest’s behavior that is causing concern about the safety of that individual and/or others in the university community but has not escalated to an emergency. This includes not only single incidents, but also patterns of behavior.

If you think it might be a good idea to submit a referral form, please do so.  The Care Team encourages referrals whenever there is doubt. However, do not use the referral form for situations that present an imminent risk of harm. If the situation is an emergency, call 911.
