Transcripts and Diplomas

Current Student Transcript Request

Apply on DOCS via Banner Self-Service. Click on the Student Information Menu followed by the Student Records Menu and then Request Printed/Official Transcript. Transcripts are processed free of charge. Your request should include:

  • Your name
  • Phone number where you can be reached
  • Your student ID
  • Number of transcripts needed
  • Mailing address for each transcript


    Alumni Transcript Requests

    Complete the Transcript Request form and send via facsimile, U.S. mail or email registrar@neomed.edu with the information listed below. No verbal requests via the telephone will be accepted.  Transcripts are processed free of charge.

    • Your name
    • Phone number where you can be reached
    • Personal identifier such as Banner ID or birthdate
    • Graduation year
    • Number of transcripts needed
    • Mailing address for each transcript
    • Additional materials (i.e. MSPE/Dean’s Letter/PSPE, certified diploma


    Transcripts processed after the University’s name change to Northeast Ohio Medical University will have the new name on the transcript with a notation on the transcript key (reverse side) of the name change and effective date.

    M.D. or Pharm.D. Student Performance Evaluation Request - Alumni (Previously known as Dean's Letter Request*)

    Requests will be accepted via facsimile, U.S. mail or email jgreen1@neomed.edu, and processed free of charge. No verbal requests via the telephone will be accepted.  Complete the M.D.or Pharm.D. Student Performance Evaluation Request or provide a written request that includes the following:

    • Your name
    • Phone number where you can be reached
    • Personal identifier such as Banner ID or birthdate
    • Graduation year
    • Number of MSPE’s needed
    • Mailing address for each (copies will not be mailed to your home for personal use)


    *For classes of 1981 through 2002

      Certified Diplomas - Alumni

      Only written requests will be accepted via facsimile, U.S. mail or email registrar@neomed.edu. No verbal requests via the telephone will be accepted. Certified diplomas are processed free of charge.

      Your request should include the following:

      • Your name
      • Phone number where you can be reached
      • Personal identifier such as Banner ID or birthdate
      • Graduation year
      • Number of certified diplomas needed
      • Mailing address for each

      Replacement Diplomas

      In the event that the original diploma has been lost or destroyed, a replacement diploma may be ordered. The graduate requesting the new diploma must submit an original notarized statement explaining the reason for the request. Access the Diploma Replacement Form here. If the replacement diploma is required due to the condition of the original diploma, the original must be surrendered to NEOMED prior to a replacement being ordered. There is a $15 fee for the replacement.  Payment may be made by check, money order payable to NEOMED or credit card. Replacement diplomas are sent in approximately 30 days.

      For graduates who seek to replace their diploma, the new diploma will bear the current name of the University and signatures of the current administration. It will be stamped “official replacement.” Diplomas will bear the graduate's name that is on file at the time of their graduation.
