Transcripts & Diplomas

Ordering transcripts through Parchment

We have a service in place called Parchment that will allow you to order digitally secure e-transcripts and/or mailed paper transcripts. Before you place your order, please read the information below about our partnership with Parchment.

Official Transcript Requests

An Official Transcript Order Request can be sent to you directly, or it can be sent to a third party of your choosing. Typically, this is for a new job requirement, licensure renewal, admission to another University or for a scholarship or membership requirement. Current students have the option to view your unofficial transcript via Banner Self-Service free of charge. The unofficial transcript is not accepted by third parties.

Transcripts will not be withheld for any employment-related requests or to apply for enrollment at another institution of higher education in the event a student seeks to transfer or pursue an additional degree or take a pre-approved course for transfer credit.

Electronic transcripts may be requested for $10 and paper mailed transcripts for $12 plus shipping.

Please note you do not need to place an order for VSLO/ERAS/MIDUS requests; please see below how to make a request for those.

Transcripts processed after the University’s name change to Northeast Ohio Medical University will have the new name on the transcript with a notation on the transcript key (reverse side) of the name change and effective date.

Students may request a paper transcript to be picked up on campus by e-mailing registrar@neomed.edu. These requests are processed free of charge. These requests may take 2-5 business days. A Registrar staff member will email you when the order is ready for pick up. The Office of the Registrar is located by the main entrance in R-121.

Request a transcript with a NEOMED login

Login to Banner Self-Service. Click on the ‘Student Information Menu’ followed by the ‘Student Records Menu’ and then ‘Place Order for Official Transcript’. You will be directed to the order screen to make your request(s).

Current students may view unofficial transcripts on Banner Self-Service.

Request a transcript without a NEOMED login

Follow this link to the Parchment site. The first time you enter the Parchment portal, you will need to set up an account, following this initial set up, you will have a login to place any future orders.

Third Party Requester – Requests made on behalf of a student/former student

If you are requesting a transcript on behalf of the student or former student, please use this link to the Parchment site. You will be required to upload a FERPA release within the portal. You may use your own FERPA authorization form or you can use this Credential Request Authorization Form.

VSAS/VSLO/MIDUS/ERAS

If you need an updated transcript uploaded to your VSAS/VSLO or ERAS account please e-mail registrar@neomed.edu.

If you need a transcript and/or your MSPE uploaded to MIDUS, please follow the steps below:

Steps for Submitting MIDUS Requests in EFDO On-line Services

  1. Log in to MyERAS and select “Additional Documents” from the Documents section.
  2. Click EFDO On-line Services, at the bottom of the “Additional Documents” page.
  3. Click MIDUS Request at the top.
  4. Once you have carefully read the disclosures, check the box next to “I have read and accept the above disclosures.” and click Continue.
  5. Read the instructions and click New Document Request.
  6. Select which document type you are requesting (MSPE or medical school transcript). You must submit a separate request for each document type you would like uploaded through MIDUS.
  7. Enter complete contact information for your medical school official.  **Please note: It is very important to enter correct contact information so that your school receives your email request and so that EFDO can contact the institution, if necessary.
  8. Enter any comments or special instructions for your medical school.
  9. Carefully check the information you supplied and click Submit. This will send the email request to your medical school.
  10. If your request was submitted successfully, you will see a confirmation page. Click OK on this page to return to the request homepage.

Medical Student Performance Evaluation (MSPE) Request

(Previously known as Dean’s Letter Request*)

The release date for the MSPE is October 1 of the M4 year. Due to the nature of this document, requests for copies by the student are only honored after the internship/residency selection process, for which the MSPE was intended, has concluded and are stamped “issued to student.” It is important to note that the transmission of the MSPE to residency programs (even after graduation) must be
directly from the institution to the residency program through ERAS. If a student is applying to residency positions outside of ERAS, the MSPE must still be transmitted directly from the institution to the program. Fellowship applications can use the secure link via MIDUS for secure MSPE sharing.

Requests should be made by using the Student Record Request Form and emailing the completed form to registrar@neomed.edu. No verbal requests via the telephone will be accepted.

Certified Diplomas

A certified diploma is a true and official copy of the graduate’s diploma issued by the Registrar’s Office.

Graduates prior to the Class of 2020

Certified diplomas are official copies of the graduate’s diploma. They include a red “issued to” stamp and a raised university seal. Requests should be made by using the Student Record Request Form and emailing the completed form to registrar@neomed.edu.  No verbal requests via the telephone will be accepted.

Graduates of the Class of 2020 and beyond

Beginning with the Class of 2020, students will receive a digital diploma in addition to their original printed diploma. These digital diplomas come with Parchment’s Blue Ribbon seal and are secure. A digital diploma will be delivered to the graduate’s NEOMED email address once the degree has been conferred.*  The digital diploma will always be accessible through the alum’s Parchment Credential Profile. It can be downloaded and shared again and again — at no cost, by logging into your Parchment account with your NEOMED credentials.

* Please note, degree conferral requires: full graduation clearance from all university offices, all final grades to be posted, no holds on the student’s account and a completed degree audit. This process can take up to 45 days from the end of the semester, though we work to confer degrees as quickly as possible.

Replacement Diplomas

In the event that the original diploma has been lost or destroyed, a replacement diploma may be ordered.  There is a $50 fee for the replacement.

For graduates who seek to replace their diploma, the new diploma will bear the current name of the University and signatures of the current administration. Diplomas will bear the graduate’s name that is on file at the time of their graduation, unless proper name change documents have been provided to The Office of the Registrar.

How to make a name change

To ensure that the University maintains accurate student records, current students who change their personal information (name, marital status, permanent or local address, permanent or local telephone number, emergency contact, citizenship status) must notify the Office of the Registrar of this change in writing. Many of these items can also be updated in Banner Self-Service by the student.

Two forms of official documentation are needed to make an official name change to a current or former student record. Acceptable documentation includes a marriage license or court ordered document and one of the following: state ID/driver’s license, original social security card, or U.S. Passport. The official documentation can be provided to the Office of the Registrar. In the case of a name change following graduation and a request for a new diploma is desired, a request for a replacement diploma can be made via the NEOMED Parchment Portal.
